Payment must be included with Registration. If attending, please include a check or money order. (Sorry, no purchase orders, vouchers or credit cards can be accepted.) If payment submission will be sent by your accounting office, please request your name be on the check stub and a copy of your registration is included with the check.

Requests for refunds must be sent to the Treasurer in writing by September 15, 2012. Substitution of attendees may be made at any time.
